Re-examination Application Instructions

The re-examination application is to be used by candidates who have already filed an Initial Exam Application and want to take additional sections of the exam.  If you have not submitted an initial application, please download the CBT Examination Application (PDF Format - 821 K) which can be completed on-line and printed for submission to the Board for approval.

Candidates who have already submitted an initial application and been issued an Authorization to Test (ATT) and wish to take additional sections may download the
Reexamination Application (PDF Format - 800K) which can be completed on-line and printed out for submission to the Board office.
